You probably won't be able to answer every question when you first visit your account.
|
|
|
| |
|
|
So that you can fill out the form at your leisure (and ultimately with any help you may need from your regular doctor), LIFECARTE system allows you to return and add to it as often as necessary before finalising your registration. All you need is your login and password. The card will be created after this step, and you will receive it within two weeks. |

So that you can prepare your answers and collect the necessary information, here is a list of the questions you will be asked to answer: |

- the names and telephone numbers of three people you would like to be contacted,
- the name, telephone number and address of your regular doctor,
- your insurance company, telephone number (hotline) and your contract number,
- your health insurance company, telephone number and your contract number,
- your assistance company, hotline number and your contract number,
- your choice with regard to organ donation,
- your blood group. |
